Invitation only event:

General practitioners, nurses and practice managers from Health Care Homes practices are invited to join pharmacists in a 2 hour workshop

The workshop will be co-presented by the Pharmacy Guild of Australia and the Pharmaceutical Society of Australia in conjunction with the Tasmania Primary Health Network.

The session will provide:

  • A briefing on the Community Pharmacy in Health Care Homes Trial Program
  • Information on the process to refer patients to the Program
  • Information on the benefits of the Program for patients
  • An interprofessional networking opportunity to discuss the Program in an open forum
