Introducing Tasmania’s ‘care finder’ service providers

Some older people need extra, intensive support to access aged care services and other supports in the community.

The Australian Government-funded care finder program helps vulnerable older people find and connect with aged care services in their local area.

Primary Health Tasmania has engaged four organisations to deliver care finder services in Tasmania.

The Council on the Ageing (COTA) Tasmania will provide care finder services to eligible older Tasmanians across the state, with interim services starting immediately and being scaled up to full service delivery in coming months.

CatholicCare Tasmania, The Salvation Army Tasmania and Wintringham have transitioned from the former Assistance with Care and Housing program and are now providing care finder services in particular parts of the state.
