After-hours toolkit for aged care facilities

Primary Health Tasmania has launched a new suite of resources to help aged care facilities manage the health care of residents in the after-hours period.
The new resources include a care planning guide, a workbook to document a facility’s after-hours plan, and a workbook to document an individual resident’s after-hours care plan. Instructional videos for each resource are also available.
The resources were developed specifically for Tasmanian facilities, in consultation with Tasmanian facilities.
