A mental health continuum of care
Primary Health Tasmania’s Mental Health Continuum of Care Project aims to improve Tasmanians’ experience of and outcomes from the mental health services we commission.
The project will seek to enhance service integration and coordination, so people experience a more cohesive journey across mental health services and receive the right care at the right time.
It will involve gathering information and feedback about our current service model to help inform a new commissioning model.
Our focus will initially be on the following types of service we commission:
- low intensity services (for people with mild mental illness or at risk of mental illness)
- short-term trauma counselling
- short-term psychological interventions
- services for adults with complex and severe mental illness.
We want to hear from you
Primary Health Tasmania is committed to an inclusive development process for the new model. We believe the best outcomes are achieved when the development of new models is collaborative.
We feel that Tasmanian Health Service clinicians who provide supports as part of Statewide Mental Health Services, can offer important insights and perspectives about the current mental health landscape in Tasmania. The consultation session will focus on:
- understanding your experiences of providing mental health services in Tasmania
- exploring the key challenges and barriers to providing and referring into mental health services in Tasmania
- exploring your priorities and ideas about improving mental health services in Tasmania.
The 60-min online consultation for Tasmanian Department of Health clinicians is scheduled for: Wednesday 14 May from 12:00pm
