My Health Record

My Health Record is a secure online summary of a person’s significant health information.

Every Australian who didn’t choose to opt out has now been given a My Health Record.

What are the benefits to providers?

Through the My Health Record system you will access timely information about your patients such as shared health summaries, discharge summaries, prescription and dispense records, pathology reports and diagnostic imaging reports.

The key benefits of the My Health Record system are:

• avoid adverse drug events
• enhanced patient self-management
• improvements in patient outcomes
• reduce time gathering information
• avoided duplication of services.

“It gives me peace of mind that their accurate information will be accessible if it’s needed – especially after hours or in an emergency situation. I know that their accurate list of past medical history, and medications and allergies is all there if it’s needed.”

What type of information can it contain?

My Health Record can contain clinical, Medicare and personal information, such as:

  • shared health summary
  • event summaries
  • hospital discharge summaries
  • specialist letters
  • eReferral letters
  • prescription and dispensing information
  • patient-entered personal health summaries
  • health notes entered by the patient
  • advanced care planning documents.

Primary Health Networks (PHNs) across the country are supporting healthcare providers including general practice, pharmacists, specialist and allied health professionals by raising awareness and delivering educational activities.
