Updated resource: After-hours toolkit for aged care homes

We have updated our after-hours support planning toolkit for aged care homes to reflect the revised Strengthening the Aged Care Quality Standards, which come into effect on 1 November 2025.

After-hours plans identify how to manage residents’ health care in the after-hours period and increase awareness of support available in primary health care (including general practice and pharmacies).

This toolkit includes a planning guide and two workbooks plus accompanying instructional videos. The resources were developed specifically for Tasmanian facilities, in consultation with Tasmanian facilities.
