New resources to support young people with intellectual disability

Primary Health Tasmania has developed a suite of resources to help young people with intellectual disability move from paediatric to adult health services.
The period of moving to adult health services is an important time to ensure young people receive appropriate support and are connected to the right services to help manage and maintain their health.
Primary Health Tasmania has worked with experts including people with lived experience, advocates, peak bodies, and health professionals to develop a framework to support the transition of young people with intellectual disability to adult health care. This new framework complements the Moving on Up Framework.
The transition framework features five key stages:
- Discuss – introducing the topic of transitioning to adult health services
- Understand – knowing everyone’s role in transitioning
- Plan – making a plan to transition
- Prepare – getting ready to transition
- Transition – transitioning to adult health services.
An overview of the transition journey and accompanying resources/links to further information have been developed for:
- young people with intellectual disability
- parents/carers
- health professionals.
It is intended that these groups will access the resources together, reflecting on the role they will each play during the transition.
