Primary Health Tasmania is responsible for improving the efficiency, effectiveness and coordination of primary care.
We support general practice – as the cornerstone of the health care system – and other community-based providers to deliver the best possible care for Tasmanians, particularly those most at risk of poor health outcomes.
We help ensure streamlined care for people moving through the health, aged care and social care systems. We do this by joining the dots between health services, linking health care providers and supporting collaboration and communication.

Primary Health Tasmania created the Nurses’ Portal in response to a need identified by the Tasmanian Nurse Leadership Group. The group found many primary health care nurses are geographically and professionally isolated and find it difficult to access education, resources and support. The Nurses’ Portal is an ‘online community’ where information, ideas and experiences are shared. You can make comments ‘publicly’ so that other users learn from your ideas. You can also message other members privately.
