For health professionals

Primary Health Tasmania is responsible for improving the efficiency, effectiveness and coordination of primary care.

We support general practice – as the cornerstone of the health care system – and other community-based providers to deliver the best possible care for Tasmanians, particularly those most at risk of poor health outcomes.

We help ensure streamlined care for people moving through the health, aged care and social care systems. We do this by joining the dots between health services, linking health care providers and supporting collaboration and communication.

Primary Health Tasmania provides a range of programs and services to health professionals and the broader community.

This website offers resources, templatesevent informationprovider directories, newsletters and news to support health professionals.

Please contact us if you have any questions or suggestions about our website and the information and resources provided for health professionals.

Primary Health Tasmania is also engaging with health professionals to identify the opportunities to improve the health and wellbeing of Tasmanians. For more information, go to our Getting involved page.
